Many companies find that their practices and procedures exist only in the heads of the individuals who are doing the job. Suddenly that person quits or gets sick and no one knows how things are done. Or something falls between the cracks and a crisis ensues.

Both these scenarios, as well as others, indicate that more formalized policies and procedures need to be updated or developed and documented.
